Activities Forms
Students in 7th and 9th grade must have physicals completed May 1, 2020, or later. Students in 10-12th grade and 8th-graders may use the physical from the previous year if it is on file at AC. All paperwork must be completed before participation in activities at Adams Central. Please read the NSAA letter regarding the change in physicals for this upcoming school year.
